We do not sell or rent your personal information for money. We do share limited technical and contact identifiers with advertising and analytics providers, which is described in detail in Section 4.
3. Patient Forms and HIPAA Compliance
Any patient-related information submitted through our website is handled in accordance with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).
Our website form includes a free-text message box, and calls to our advertising numbers may be recorded. You may therefore share health details through those channels. We treat anything you share that way as protected health information. You are never required to describe symptoms or treatment to reach us: a name and a preferred time are enough, and we will gather clinical details through a secure channel.
4. Analytics, Advertising, and Tracking Technologies
We use analytics and advertising technologies to understand how our website is used and to measure our marketing. These technologies place cookies or similar identifiers on your device.
Google Analytics
We use Google Analytics. For each page you view, it sends Google the full address and title of that page, together with a long-lived identifier stored in cookies named _ga and _ga_JWR551LKGK, plus your approximate location, device, browser and language. Because our page addresses and titles name the treatments we offer, this record can indicate which dental services you were reading about. The information is collected visit by visit, tied to that cookie identifier, and reviewed by us in aggregate.
Google reCAPTCHA
Our forms use Google reCAPTCHA to block automated submissions. It runs on pages containing a form and sends Google device and behavior signals to score whether you are human. Its use is governed by Google’s privacy policy and terms.
Meta (Facebook and Instagram) Pixel
The Meta Pixel runs on this website. It sets cookies (including _fbp and _fbc) and sends Meta your IP address, browser and device information, and a record that you visited our site.
At present the Pixel reports only our website’s main address to Meta and not the specific page you are viewing, so it does not currently tell Meta which treatment pages you read. This behavior is controlled by Meta, not by us, and could change.
We also record when someone selects an appointment booking link, so we can count how many people begin booking. That record contains no name or contact details.
Meta Conversions API (information sent from our server)
When you submit a request through our website, we send Meta a record of that submission directly from our servers, not from your browser. That record contains a scrambled (SHA-256) form of the contact details you entered, along with your IP address, browser information, and our website’s main address. We deliberately do not include the specific page you submitted from, and we never send the free-text message you typed.
Please understand what the scrambling does and does not do. It prevents others from reading your details in transit. It does not make you anonymous to Meta: the purpose of sending it is so Meta can compare that scrambled value against its own records and determine whether you hold a Facebook or Instagram account. Scrambling of this kind is not unbreakable.
Your choices, stated plainly
These two paths are not equally controllable, and we would rather say so than imply otherwise:
- Browser-based tracking (Google Analytics, reCAPTCHA, the Meta Pixel) can be limited using your browser’s cookie and tracking settings, your Google and Meta account ad settings, or industry tools such as the Digital Advertising Alliance (optout.aboutads.info) and the Network Advertising Initiative (optout.networkadvertising.org).
- The server-to-server record described above cannot be stopped by any of those settings, because it is sent by our website rather than by your browser. If you would prefer that no such record is created, please book by telephone at (801) 491-6844 or email us at info@mapletonfamilydentistry.com instead of using the website form. You may also contact us at that address to ask us to delete information we hold about you.
Declining any of these technologies does not affect your ability to receive care.
5. Call Tracking and Call Recording
The telephone number published on this website is our main office line. We do not use dynamic tracking numbers on this website, so browsing our site does not create a call record.
Separately, our advertising sometimes displays a different telephone number that forwards to our office. It is used only to tell us which advertisement produced a call. When you dial one of those advertising numbers we may record your telephone number, the date, time and duration of the call, whether it was answered, and which advertisement referred you.
Calls placed to an advertising number may be recorded. A notice is played before the call connects. Recordings are used for quality assurance, staff training, and confirming scheduling details. If you would prefer not to be recorded, call our main office line at (801) 491-6844, which is not recorded, or email info@mapletonfamilydentistry.com.
Call records are held by our call tracking provider on our behalf and are not sold, and not shared for anyone else’s marketing. Please avoid leaving detailed health information on a recorded line or in a voicemail; our team will collect what is needed through an appropriate channel.
6. Online Scheduling Through a Third-Party Platform
Appointment booking on this website is provided by Modento, a third-party scheduling platform. When you select an appointment or booking link, you leave our website and continue on a Modento web address (book.modento.io).
Any information you enter on that platform, including your name, contact details, and the appointment details you choose, is collected by Modento and handled under Modento’s own privacy policy and terms. We receive the resulting booking information so that we can prepare for your visit.
We add campaign tags to booking links so we can understand which marketing efforts lead to appointments. These tags describe the marketing source only. They do not contain your name or contact details.

7. Marketing Communications
If you provide your email address or phone number, we may send you occasional marketing or informational messages related to Mapleton Family Dentistry.

We may share your personal information, including your mobile phone number, SMS opt-in data, and consent status, with third parties that help us provide our messaging services, including platform providers, phone companies, carriers, compliance providers, and other vendors who assist us in the delivery, management, or support of text messages. These providers may use that information only to deliver messaging services on our behalf.
Consent categories
- Transactional messages — appointment reminders, appointment confirmations, scheduling updates, and replies to requests you submit — are sent to patients and prospective patients who have provided a mobile number and opted in.
- Marketing messages — promotional offers, review invitations, and feedback surveys — require a separate, optional opt-in. Marketing opt-in is never pre-selected and is not a condition of receiving dental care, goods, or services from Mapleton Family Dentistry.
Message frequency varies. Message and data rates may apply. Delivery of text messages is subject to transmission by your wireless carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages.
You may opt out of receiving SMS communications at any time by replying STOP to any message, or by contacting us at (801) 491-6844 or info@mapletonfamilydentistry.com. Upon receipt of your opt-out request, we will promptly stop sending SMS messages to your number. Reply HELP at any time for assistance, or contact us using the phone number or email address above.

All third parties are required to maintain appropriate privacy and security protections. Mobile phone numbers, SMS opt-in data, and SMS consent status are never shared with third parties or affiliates for their own marketing or promotional purposes. The service providers we use in operating this website and our marketing include Google (analytics), CallRail (call tracking for advertising numbers), Modento (online scheduling), and Twilio (text messaging). Each receives only the information needed to perform its service for us. Meta Platforms is different and we list it separately: we send Meta advertising information as described in Section 4, and Meta also uses that information for its own advertising optimization, not solely on our behalf.

We take reasonable administrative, technical, and physical measures to protect your information from unauthorized access, disclosure, or misuse.
